Ideal Clinical Liaison candidates should also have:


Assist referral sources in identifying patients with care needs that match services provided. Educates potential patient and family regarding the services provided, facilitates the admissions process, communicates with appropriate referral source staff and ensure s accommodations and staff are prepared for the patient.

Essential Job Functions


  • Receive referral inquiry from providers, facilities or any customer source

  • Provide on-site clinical assessment and assist referral sources with identification of appropriate patients including education regarding admissions criteria


Qualifications

- Registered Nurse in good standing currently licensed by the State in which the services are prepared
- Must have current certification for CPR
- Must be able to relate positively and favorably to residents and families and to work
- Basic computer skills to include electronic mail and Microsoft Office
- Knowledge of third party reimbursement
- Self directed and able to work with minimal supervision
- Excellent verbal and written communication skills
- Must be able to maintain confidentiality regarding patient and company proprietary information
- Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
