As a Patient Care Coordinator at AseraCare Hospice, you’ll be responsible for daily management of nursing and patient care. Rather than treating the disease, you’ll focus your staff’s efforts on the whole person. With so many relying on your clinical leadership, direction and supervision, you’ll help our patients live life to the fullest. You’ll be an advocate for patients and families, maintaining existing programs and creating new ones to meet their special needs. Provide invaluable support for our patients and families during these difficult but significant moments in their lives as a Patient Care Coordinator by performing a wide variety of daily responsibilities.

Ideal Patient Care Coordinator candidates should also have: 

Responsible for the day-to-day management of patient care activities and supervision of clinical staff. Participates in quality improvement program and the evaluation of nursing and other clinical services. Supports the Provider Relations and volunteer activities of the organization.

Essential Job Functions

  • Supervises patient care team in the delivery of care. Supervises and assesses the quality of contracted services in relation to patient care. Receives and assigns case referralsto personnel


  • Assist director of clinical services in ensuring that all patient and family services comply with federal, state(s) and local regulatory requirements and with accepted professional standards


  • Participates in the development, implementation and evaluation of policies and procedures for patient and family services. Participates in the selection and evaluation of all direct patient and family care staff. Participates in the orientation and ongoing education of staff and volunteers


  • Coordinates the development, implementation and reviewof the interdisciplinary plan of care for each patient and family. Communicates as needed with all members of the team, the patient and family, the medical director, the administrator, and others involved in the care of the patient to facilitate optimal care of patients/families. Initiates preventive and rehabilitative nursing procedures. Reports changesin the patient’s medical and mental condition to the attending physician as well as to other staff and contract employees involved in the patient’s care

  • Maintains the clinical records, including nursing clinical notes and progress notes, for each service provided to each patient according to established standards and agency policy. Assists with the processing and maintenance of all reports required for third-party billing. Conducts quarterly record reviews and communicates findings and recommendations to director of clinical services and staff

  • Assures the 24-hour availability of nursing services to patient/families and assists director of clinical services in assignment of nursing personnel. Participates in twenty-four hour, seven days per week call, as scheduled by the DOCS. Follow throughwith specific plan of care as established and modified by the interdisciplinary team with his/her individual comments and suggestions incorporated

  • Teaches patient and family members regarding nursing care skills, procedures, techniques and use of equipment


  • Participates in quality improvement reviews especially related to the evaluation of nursing and contracted services. Make recommendations for improving and enhancing the overallfunctioning of the agency


  • Manages the development of a discharge plan when a patient is dischargedprior to death or transferred to another provider


  • Participates in public relations and community activities that promote the organization. Promotes customer service orientation to all personnel

  • May be expected to perform on-call professional duties


- Registered nurse with a current state license
- Minimum two (2) years experience as staff nurse in an acute care hospital or home health agency
- Minimum one (1) year supervisory experience
- Must have 24-hour access to a reliable motor vehicle and must maintain a valid driver's license and maintain automobile insurance that complies with state and company regulations
- Must be capable of maintaining regular attendance
- Ability to assess and respond to the needs of patients and families in a home setting
- Skilled in nursing practices
- Ability to cope with family emotional stress
- Tolerant of individual lifestyles
- Must be able to maintain confidentiality regarding patient and company proprietary information
- Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
- Must possess computer skills at a level necessary to accomplish the job
