As a Patient Care Manager (RN) at AseraCare Hospice, we depend on you for the daily management of nursing and patient care. You'll focus your staff's efforts to care for the clinical, emotional, and spiritual needs of each patient and family member. With so many relying on your clinical leadership, direction and supervision, you'll help our patients through their end of life journey. You'll be an advocate for patients and families, ensure the highest quality of care, and meet the individual and unique needs of our patients. If your passion is to lead those who care for our patients as they embark on this precious journey, your experience as a registered nurse manager will lead our mission to make moments matter for each patient and family. QUALIFICATIONS

  • Registered Nurse with a valid state license
  • Minimum two (2) years of hospice or home health experience, preferred
  • Experience in dealing with dying patients and their families with a strong understanding of pain and symptom management (required)
  • Minimum one (1) year supervisory experience (required)
  • Must have 24-hour access to a reliable motor vehicle and must maintain a valid driver’s license and maintain automobile insurance that complies with state and company regulations
  • Must be capable of maintaining regular attendance
  • Intermediate skills to assess and respond to the needs of patients and families in a home setting
  • Effective communicator – both written and verbal
  • Competency in all aspects of IV therapy
  • Skilled in end-of-life nursing practices
  • Ability to listen and cope with family emotional stress
  • Must be able to maintain confidentiality regarding patient and company proprietary information
  • Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels
  • Must possess computer skills of the types and at a level necessary to accomplish the job
