You're more than a Staff Nurse LPN at AseraCare Hospice. Under the direct supervision of a registered nurse, provides continuous care services, in-home respite care services, and personal care for the patient and assists with activities of daily living. Supports the Provider Relations activities of the organization.


  • Delivers nursing services in accordance with hospice policies. Follows the interdisciplinary plan of care. Is alert to the patient's needs and concerns and teaches appropriate self-care techniques to patients and family members. Administers medications
  • Provides personal care, including: bathing, dressing, grooming, caring for hair, nail care, oral hygiene, skin care, and foot care
  • Prepares supplies and equipment for treatments that require sterile or aseptic technique in accordance with the Plan of Care
  • Assists patients in turning, getting up in a chair, or ambulation according to the plan of care. Assists with elimination, including: bedpan, urinal, up to the bathroom, routine foley care and enemas according to the plan of care. Assists patients with meals, if necessary
  • Performs household services essential to maintaining a safe and comfortable environment
  • Assists the attending physician or registered nurse in performing specialized procedures as an extension of therapeutic services, prepares equipment, supplies, and treatments that require using sterile or aseptic technique, in accordance with the interdisciplinary plan of care
  • Observes patients when giving care and reports changes in the patient's condition and needs to the Patient Care Coordinator
  • Completes appropriate clinical progress note documentation in a timely manner
  • Participates in ongoing education programs
  • Follows the policy of confidentiality regarding patients and families
  • Demonstrates a compassionate attitude toward patients and families that is consistent with the hospice philosophy and mission
  • Attends patient care conferences as requested by the Patient Care Coordinator. Contributes to team effort by accomplishing related results as needed. From time to time this position will be expected to perform on-call professional duties


  • Must adhere to the Company's Code of Conduct and Business Ethics policy including documentation and reporting responsibilities


  • Licensed Nurse with current state license
  • Minimum of one (1) year experience as a Licensed Vocational Nurse, and shall work under the supervision of a Registered Nurse.
  • Previous experience in hospice, home health, geriatrics, or general medical/surgical nursing
  • CPR certified.
  • Must demonstrate a commitment to the hospice philosophy of care.
  • Must have valid driver's license, 24-hour access to a motor vehicle and maintain personal auto liability insurance coverage
  • Must be capable of maintaining regular attendance.


  • Must be able to maintain confidentiality regarding patient and company proprietary information
  • Must be able to relate professionally and positively to corporate staff and to work cooperatively with other employees at all levels
  • Must possess computer skills of the types and at a level necessary to accomplish the job


  • Mobility, reaching, bending, lifting, grasping, fingering, fine hand coordination, ability to talk and hear; ability to read and write; ability to communicate with personnel, ability to remain calm under stress. Ability to adequately lift, support and bodily manipulate the weight of a patient.
